Streams is a mod created by delvr. It modifies vanilla Minecraft's rivers such that they have a true current and directional water flow. River water will also descend and ascend accordingly with the terrain. Waterfalls also flow into bodies of water in a much more realistic fashion, with a slight slope at the bottom rather than a simple pillar of water.
This mod is dependent on the Farseek library.
